Faculty Evaluation Forms
The forms are available two ways, as
PDF files that faculty may download, print out and then complete by hand, or
as
Word documents that may
be downloaded and then completed on your computer.
Note: Electronic forms that
require a signature (like the Summary Evaluation Form) still need to be printed
so they can be signed before being submitted.
Word Documents:
In order to download one of the documents below
to you computer, right-click on the document icon and choose "Save Target As..." from the menu.
